Facility Evaluation Report
Licensing Program Analyst (LPA) Mariana Agban conducted an Annual Required visit and inspection of the facility. LPA met with Lincesee Salvacion Butler and explained the reason for the visit. Administrator, Arnaldo Hukom arrived shortly after. At approximately 09:25 am, with the assistance of the Licensee, LPA took a tour of the physical plant. Required postings were observed in the entry area.
Kitchen: The kitchen appliances and fixtures were functional. LPA found a sufficient amount of perishable and non-perishable food at the facility; properly stored. Knives were stored in a locked kitchen cabinet.
Bedrooms: There were five (5) bedrooms designated for residents' use and one (1) room designated for staff. All five bedrooms, in use by residents, were properly furnished with appropriate bedding and linens with sufficient lighting. Bathrooms: There are three(3) bathrooms designated for residents' use. Bathrooms were properly supplied and had functional fixtures. Hot water temperature was measured from the bathroom sink at 114.4 degrees Fahrenheit. No cleaning supplies or hazardous items were present in each bathroom during the inspection. Common Areas: These included the living room and dining area. The common areas were properly furnished. The auditory alarms on all exit doors were on and functional at the time of the visit. The dining room table is large enough to sit the capacity of the facility. Seating such as couches were in good repair and sit the capacity of the facility. Properly labeled medications were locked in one of the living room cabinets. The fireplace located in the living room is closed, non-operational, and blocked off.
Surrounding Grounds: Entry/exits were free of obstruction. There is a large patio table and chairs shaded by a large umbrella for clients to use in the backyard. Patio furniture was observed to be in good repair with adequate seating for the residents. Laundry room appliances were observed to be functional. Toxins are locked and stored in the garage cabinets. The smoke alarms were tested and were functional. The fire extinguishers are located in the kitchen, dining room, and living room with a charge date of February 24, 2024. (Continue on 809C)
