On 11/25/25, at 9:32AM , Licensing Program Analyst (LPA) Gina Saucedo arrived at the facility to conduct an unannounced, case management visit and was greeted by Administrator, David Aguiniga .
The visit is notifying the licensee of this case, referencing the complaint #31-AS-20210712154639, and that the Department is in the process of assessing an additional civil penalty under H&S Code 1569.49(f) for the allegation that a resident sustained pressure injuries while in care.
