Facility Evaluation Report
Licensing Program Analyst (LPA) Mariana Agban arrived at the facility above to conduct a Case Management - Legal/Non-compliance visit and inspection to ensure compliance. LPA met with staff #1 (S1) and explained the reason for the visit. On December 13, 2022, a Non-Compliance Conference (NCC) was held at the Woodland Hills Regional office. As a result, the facility was placed on a three-year compliance plan. Administrator David Kawooya was contacted but was unable to come to the facility. During today's visit, LPA conducted a physical plant tour to ensure the health and safety of the clients and that the physical plant was in compliance with Title 22 Regulations. LPA requested staff records but was informed that they were unavailable, as only the Administrator had access to them. LPA reminded the Administrator that a designated substitute should be present at the facility when the Administrator is absent. LPA also emphasized to the Administrator that records must be available at all times for review and inspection by community care licensing representatives.
An exit interview was conducted, citations were issued, and a copy of this report was delivered.
