Facility Evaluation Report

Licensing Program Analyst (LPA) conducted a case management visit to document the visit conducted today, to deliver the Annual Inspection report generated on 2/24/26. LPA Yee met with Diana Gevorgyan, House Manager and the reason for today's visit was provided.

A required Annual Inspection was initialed on 2/24/26. A report for the required annual inspection visit conducted to the home on 2/24/26 was not provided to the facility due to an error that occurred when the CARE Inspection Tool was being finalized. No deficiencies and Technical violation reports were created and notes made in the comment fields were not generated. LPA Yee coordinated with IT to resolve the problem and conducted a return visit today.

Exit interview was conducted with Diana Gevorgyan. A copy of this report and the Annual Inspection report for the 2/24/2026 visit was provided.
