Complaint Investigation Report
#1 and Resident #4's rooms were locked and Resident #3 refused to allow LPA to view the room. Per observation of the residents, they are doing well except for the personality conflicts among the residents. Utilities were observed to be in use during the initial visit. Food supply was reviewed and sufficient perishable foods for a minimum of 2 days and insufficient non-perishable for a minimum of 7 days was observed.
Per tour of the back yard, the pool located in the backyard was secured with a rod iron fence and the gate was observed padlocked. The backyard needs to be more organized and items such as ladders need to be stored away. Per information received on initial visit, it was determined that further investigation is needed to make a finding for the above allegation. Exit interview was conducted.
On September 19, 2025, Licensing Program Analyst (LPA) Christine Yee and Investigator Rocio Flores conducted a joint subsequent complaint visit to conduct additional investigation for the above allegations. Also present during the visit were Officer Ramirez and Officer Hewitt with the Los Angeles Police Department, North Hollywood Division. The Team was let into the home by Diana Gevorgyan, Staff. Andrew Akhparian, Administrator, was advised of the team’s presence at the home and he joined in on the visit at 9:47am. The reason for subsequent visit was provided.
During the subsequent joint visit, Investigator Flores conducted interviews with the Administrator, staff and toured the residents' room, the two (2) sheds and a storage cabinet located in the backyard. A request was made to tour the ADU located on the premises but was denied access. Also discussed during this visit was the facility's policy on firearms, Copies of the following documents were requested emailed to LPA Yee and Investigator Flores by Monday - 9/22/25: the facility's firearms policy, documents for the Administrator's firearm safety training - received during visit, Staff schedule - LIC500, copy of the Personal Rights training and copy of Administrator documents for Diana Gevorgyan. Based on the information received during the visit, it was determined that further investigation is needed before a determination can be made for the above allegations. Exit interview was conducted.
