The inspector’s account

Licensing Program Analyst (LPA) Quoc Huynh arrived at the facility unannounced for a required one-year visit. LPA arrived at 12:00PM and met with the Administrator Diana Gevorgyan. Entrance interview conducted.

Beginning at 12:20PM, the LPA and Administrator conducted a tour of the physical plant to ensure residents’ health and safety, and the facility is in compliance with Title 22 regulations. The following was observed:

KITCHEN: Knives and cleaning supplies were stored inaccessible in locked cabinets and drawers. Kitchen appliances and fixtures were clean and functional. The facility also had a sufficient supply of perishable food. Food in the refrigerator and freezer were of good quality, however two (2) food items had expiration dates of 07/12/2025 and 04/23/2026. The Administrator discarded the expired food. On 04/11/2025, LPA Huynh’s annual visit noted broken drawers in the refrigerator that the Licensee agreed to fix. During today’s visit, the drawers remained broken.

COMMON AREAS: The common areas included a living room, den, and dining room. All furniture was clean and in good condition and a fireplace was observed to be properly screened. There was sufficient space to accommodate both indoor and outdoor activities. Nightlights were observed throughout the hallways. Emergency food was observed by the entrance and emergency water was located in the den. The facility had a laundry room that contained general storage and supplies, locked chemicals, machines in good condition, and a sufficient supply of non-perishables.
