Facility Evaluation Report
Licensing Program Analyst(LPA) Christine Yee conducted a required Annual Inspection using the complete CARE Inspection Tool and initially met with Stephen Manalo, Administrative Assistant. Hector Gomez, Administrator, was contacted and he arrived later to conduct the visit. The reason for today's visit was explained.
The facility is a 2 story, large commercial building consisting of bedrooms number 100-125 (26 rooms) downstairs and bedrooms #201-203(3 rooms) upstairs, dining room, 2 common shower rooms and 2 half bathrooms downstairs and 1 full bathroom and a half bathroom upstairs. The facility is fire cleared for 58 NON-AMBULATORY residents.
On today's visit the following five domains of the CARE Inspection Tool were reviewed: Operation Requirements, Staffing, Personnel Records - Training, Client Records-Incident Reports and Clients Rights -Information. Also reviewed was the Emergency Disaster Preparedness Plan, Infection Control Plan, 5 Client files and 7 staff files.
Deficiencies cited under California Code of Regulations, Title 22, Division 6, Chapter 6. Any deficiencies not addressed on today's visit will be addressed on a return visit.
Exit interview was conducted, APPEALS RIGHTS were discussed and a copy was given.
