Licensing Program Analyst Trevor Byrne created this report to follow up on a substantiated allegation of a complaint investigation. The facility closed on August 1, 2025. The LPA delivered the report via e-mail, first class mail, and certified mail.

On January 7, 2026, the Department concluded a complaint investigation regarding the following allegation: Staff physically abused residents and staff verbally abused residents.

The licensee was cited for California Code of Regulations (CCR) 87468.1(a)(3) Additional Personal Rights of Residents in Privately Operated Facilities and CCR 87468.1(a)(1) Additional Personal Rights of Residents in Privately Operated Facilities.

At the time of the complaint visit on January 7, 2026 , an immediate civil penalty of $500 was issued and the licensee was informed that an additional civil penalty might be assessed based on Health and Safety Code § 1569.49(f).

The Department has concluded an analysis and has determined that a civil penalty is warranted for physical abuse. The Welfare and Institutions Code Section 15610.63 for physical abuse means any of the following: (a) Assault, as defined in Section 240 of the Penal Code. This is evidenced by staff (S1) striking a resident (R1) with a closed fist five (5) times on their left temple resulting in discoloration on the left side of R1’s face. Additional interviews with other residents (R2 and R6), revealed that S1 had also struck them in the past.
