Facility Evaluation Report

Licensing Program Analysts (LPAs) Trevor Bryne, and Sandra Urena, along with Licensing Program Managers (LPMs) KaSandra Lopez, and Ulysses Coronel, and Regional Manager (RM) Benita Yates conducted an unannounced case management visit at this facility. The purpose of the visit is to issue a Temporary Suspension Order (TSO). The TSO effective date is the close of business hour today. RM, LPMs, and LPAs met with facility staff Tatyana Sevostyanova. Facility staff contacted the facility Administrator Elba Khachatryan via telephone call. The Administrator did not arrive to the facility during today's visit, although RM Yates spoke with Administrator Elba Khachatryan during the inspection. During today’s visit Elba Khachatryan was located at another assisted living facility, Rossmoyne Hills. Elba Khachatryan was served the TSO for Silver Light Care, INC. at Rossmoyne Hills. A copy of the TSO was left at the facility.

A Temporary Manager is being implemented to allow placement agencies sufficient time to assist with placement of the six (6) residents. The Temporary Manager arrived during the visit, to take over operation of the facility.

During today's site visit, the RM also left copies of the "Order to Individual for Immediate Exclusion from All Facilities" for Edgar Khachatryan and Knarik Panosyan . It was determined by the Department that a complaint of conduct inimical was substantiated against Edgar Khachatryan and Knarik Panosyan , thus, necessitating the Order for Immediate Exclusion from the facility. Today, RM left copies of the Immediate Exclusion Orders for Silver Light Care regarding Edgar Khachatryan and Knarik Panosyan, along with a copy of the “Order to Individual for Immediate Exclusion from All Facilities" at the facility.
