Facility Evaluation Report
Licensing Program Analyst (LPA) Trevor Byrne arrived to the facility at 01:46 PM to conduct an unannounced Case Management – Other inspection at the facility today. LPA met with facility staff Tatyana Sevostyanova. Facility staff contacted the facility Administrator Elba Khachatryan via telephone call. The facility Administrator was unable to come to the facility at the time of the investigation but has designated facility staff to sign this report on their behalf. Entrance interview conducted.
During today’s visit LPA conducted a physical plant tour and interviewed six (6) of six (6) residents. No immediate health and safety concerns were observed. LPA observed the facility to be clean and supplied with sufficient amounts of food. All residents interviewed stated that they are fed well, staff are doing a good job, and medications are given appropriately. No residents interviewed had concerns with the facility at the time of the inspection. LPA briefly discussed the inspection authority of the licensing agency with the Administrator during today’s visit.
No deficiencies were observed during today’s inspection. This report was read to the Administrator via telephone call. Exit interview conducted and a copy of the report was issued.
