Facility Evaluation Report
Licensing Program Analyst(LPA) Christine Yee conducted an unannounced case management visit due to the deficiencies observed during a visit to the facility today. Asmik Nadrian, Administrator was not able to conduct today's visit and sent over a friend of a resident, Arshalouis Manoukyan to conduct the visit. The reason for today's visit was explained.
The following deficiencies were noted on today's visit:
the friend of the resident, Arshalouis Manoukyan, who was present at the facility to act as the designated representative of the Administrator and to conduct the visit at the request of the Administrator does not have a criminal record clearance.
files requested on today's visit could not be provided as the key to the locked cabinet containing the residents and staff files were with the Administrator. Staff present at the facility do not have keys or access to the files.
Deficiencies cited under California Code of Regulations, Title 22, Division 6, Chapter 8. Immediate CIVIL PENALTIES were assessed.
Exit interview was conducted, APPEALS RIGHTS were discussed and a copy was provided
