Facility Evaluation Report

Licensing Program Analyst (LPA) Ashley Smith conducted an unannounced Case Management- Deficiencies visit at the facility today due to deficiencies observed during a subsequent complaint visit for complaint control # 29-AS-20211221131518.

During today's visit, the LPA was initially greeted by staff Mariam Baghdoyan. Ms. Baghdoyan explained that they had recently began working at this facility. During today’s visit, the LPA checked the Department clearance list and identified that Ms. Baghdoyan was working at this facility without a clearance transfer. Administrator Asmik Nadrian confirmed that they had yet to transfer the staff's fingerprints to this location.

The following deficiencies were observed (See LIC 809-D.) and cited from the CA Code of Regulations, Title 22 and California Health and Safety Code. Failure to correct the deficiencies may result in civil penalties. Civil penalties assessed. Exit interview conducted with the Administrator over the phone. Copy of the report and appeal rights were issued. Staff were authorized to sign the report.
