Licensing Program Analyst (LPA) conducted an unannounced case management visit due to the deficiencies noted on a visit to the facility today. The reason for the visit was provided.
During a visit to the facility to conduct a Prelicensing and Component III with the new owner, it was observed that Danil Stukov, Staff, hired on 7/1/25, has received a criminal record clearance but is not associated to the home.
Deficiencies were cited under California Code of Regulations, Title 2, Division 6, Chapter 8. Civil Penalties were assessed.
