not be at the facility until 2pm but arrived at the facility at 1:10pm to conduct the visit. The reason for the visit was explained.

On the visit conducted on 9/12/24, LPA Yee conducted a tour of the private bathroom located inside Bedroom #3 at 12:25pm, the kitchen where the cordless facility phone is located at 12:29pm, a call was made to the facility line at 1:08pm, obtained copies of Resident #1's documents at 1:15pm, interviewed Staff #1 at 12:33pm, Staff #2 at 3:22pm and Administrator at 3:49pm. Based on information received on 9/12/24, further investigation is needed to make a finding for the allegations. Exit interview was conducted and a report was provided.

Prior to today’s visit, LPA conducted a telephone interview with Resident #1, who no longer resides at the facility on 2/19/25. On today’s visit, LPA Yee conducted an interview with Resident #2 at 3:58pm, Resident #3 at 3:59pm, Resident #4 at 4:02pm and Resident #7 at 4:06pm. Per interviews conducted, Resident #2, Resident #4 and Resident #7 have cell phones. However, Resident #7 prefers to use the facility phone since it is easier to dial and has not had any issues having access to the phone. Resident #3 only receives calls and has never made any outgoing calls, Residents #5 and #6 could not be interviewed. All residents interviewed indicated that they have not had any issues accessing the facility except for Resident #1. One reason provided by Resident #1 as to why they don't have access to the phone is because it is in use. Per interviews conducted with staff, the cordless phone, located in the kitchen is handed to the residents if they ask staff for it. Staff do not refuse to give the residents the facility phone. If the facility phone is in use and there is an emergency facility will allow the use of their personal cell phone to the resident. Although the allegation that Residents do not have access to a phone may have happened or may be valid, there is not a preponderance of evidence to prove that it did or did not occur, therefore the allegation is unsubstantiated at this time.

Regarding the allegation that Staff did not conduct a proper assessment, LPA Yee reviewed Resident #1's file and observed that a preplacement assessment was made on 8/22/24 using the LIC603. The Health, Mental, Social, Physical Ambulatory Status, Functional Capabilities, Services Need were completed based on the observations of a non-medical professional. Resident #1, no longer lives at the facility and no further information was provided as to how the facility did not conduct a proper assessment, therefore based on insufficient information provided, for the allegation that Staff did not conduct a proper assessment, the allegation is unsubstantiated at this time.
