Complaint Investigation Report
Party(RP) at 1:59pm, Resident #1 at 2:54pm and Staff #1 at 4:15pm. Based on information received on the initial visit, further investigation is needed to make a finding for the above allegations. Exit interview was conducted.
Per interview conducted with Staff #1 regarding Allegation #1 – Staff hit resident, during the initial visit, the police were at the facility on 2/23/24 around 7pm. Per Staff #1, who does not speak much English, they called Marine Arshakyan, Staff. The police spoke with Marine and Resident #1. Resident #1 denied calling the police and they apologized and left the home. Per information received from interviews conducted with Resident #1, either Staff #1 or Staff #2 grabbed them roughly but was not sure if they hit them. Resident #1 also said later in the interview, that staff hit them on their shoulder and arm. The staff who did the hitting was not specifically identified by the resident. LPA was being given conflicting information, Resident #1 was specifically asked if they were hit by staff and resident did not respond. Per Resident #1, they are constantly afraid of staff. Staff #1 is “good” and Staff #2 is nasty. Per Resident #1, Staff are physically and verbally abusive and they are afraid of retaliation. Per interviews conducted today with Staff #1 at 1:01pm and Staff #2 at 1:04pm, they both deny ever hitting, yelling or grabbing residents. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove that the violation did or did not occur, therefore the allegation that staff hit resident is deemed unsubstantiated at this time.
