Facility Evaluation Report
Licensing Program Analyst (LPA) Esther Cortez arrived at the facility unannounced to conduct a required annual inspection. LPA arrived at the facility at 10:00AM and met with staff and explained the reason for the visit. Administrator Parandzem Kostanyan was called via phone and was explained the purpose of the visit. The administrator arrived shortly thereafter. Entrance interview conducted.
LPA Cortez and staff conducted a tour of the physical plant at 10:25AM to ensure there are no health and safety hazards and facility is in compliance with Title 22 Regulations. The facility is a Regional Center Level 3 one story residential home. The following was observed:
KITCHEN: Knives and other sharps are stored inaccessible in a locked drawer near the pantry. The pantry contained non-perishable foods as well as emergency food and water. The facility has enough supply of perishable and non-perishable food. Kitchen appliances were in operable condition. Freezer and refrigerator were stocked with a variety of foods and labeled. Licensee stated they replenish foods every week. Cleaning supplies were stored under the sink and remained locked at all times.
COMMON AREAS: At the time of the visit, living room and dining room furniture was observed to be in good condition. The facility maintained a comfortable temperature throughout the visit. The fire extinguisher was fully charged and serviced on 12/04/2025. The LPA observed required postings between the dining room table and kitchen. Sufficient lighting and night lights were observed throughout the facility.
BEDROOMS: The facility has six (6) bedrooms: one (1) staff bedroom, one (1) shared client bedroom, and four (4) private client bedrooms. Report Continued on LIC 809-C
