At 2:40 p.m. on 09/09/2023 Licensing Program Analyst (LPA) Nicholas Reed conducted an unannounced case management visit.
LPA toured the exterior of the facility at 2:45 p.m. and called the licensee at 2:50 p.m. to disclose the reason for the visit.
LPA confirmed that there were no residents residing at the facility.
