Facility Evaluation Report

Licensing Program Analyst (LPA), Tihesha Smith conducted an unannounced Required 1-year inspection at this facility at approximately 1:20 pm. LPA Smith was greeted by staff and disclosed the reason for the visit. The staff contacted the administrator. LPA Smith spoke with the administrator who revealed is out of the area and not able to come to the facility but authorized staff to sign report.

This facility is a single story home with four (4) bedrooms and two (2) bathrooms. LPA Smith conducted a tour of the home at approximately 1:40 pm.

Due to equipment issue the unannounced annual inspection will be conducted at a later date.
