Facility Evaluation Report

Licensing Program Analyst (LPA) Tihesha Smith conducted a case management visit at this facility. LPA Smith was greeted by staff and disclosed the reason for the visit. Administrator was not present at facility.

LPA Smith made a visit to this facility to do a health and safety check. Information was received that the ac in the facility is not working. LPA Smith spoke with caregiver Jaizel who states ac went out last night at approximately 10 PM and repair person to arrive on 3/19/26 between 10 am -12pm. Temperature in facility was 86 degrees. Fans have been placed in each resident room and in common areas. LPA Smith also spoke with two (2) residents in the facility. One was looking at recipe book and the other was watching TV. Both residents stated they were okay, thank you for the fans and had water cups next to them. Additional resident in room did not answer questions. Staff stated conducting frequent checks on all three residents. Administrator will follow up with LPA Smith tomorrow with status of AC repair .
