Facility Evaluation Report

Licensing Program Analyst (LPA) Tihesha Smith conducted a case management visit at this facility at 10:37 am to follow up on the past due licensing fees and change of ownership for this facility. Staff revealed the administrator was not present and contacted the administrator. The administrator Estelita (Ester) Agpaoa arrived at 11:07 am.

LPA Smith went over the annual inspection report, past due licensing fees with the administrator and requested status of the change of owner. Administrator revealed that she is aware of the past due licensing fees and will follow up with the licensee on both the fees and change of ownership. The past due fee information and change of ownership per the Dept is as follows:

Tycoon Residential anniversary date was 11/28/25. The total amount due is $4,452.00

Change of ownership: Incomplete application received on 01/15/26 under Comfort Care Homes 197611002

Missing documents: LIC 308, LIC 9283, partnership/articles of incorporation/articles of organization, Administrator certificate, neighborhood complaint policy, orientation certificate

Copy of Notice of Application submission status was left with staff as administrator not present.
