The inspector’s account

Licensing Program Analyst (LPA), Tihesha Smith conducted an unannounced Required 1-year inspection at this facility at approximately 10:55am. LPA Smith was greeted by staff and disclosed the reason for the visit. The staff contacted the interim administrator who arrived at approximately 12 p.m. and LPA Smith disclosed the purpose of the visit.

This facility is a single story home and on 11/03/25 an Emergency Approval to Operate (EAO) was granted.

LPA Smith conducted a tour of the facility plant with the administrator and one (1) caregiver.

Due to time constraints the unannounced annual inspection will be conducted at a later date.
