Facility Evaluation Report
At 11:00 a.m. on 11/03/25, Regional Manager (RM) Angela Whittaker and Licensing Program Analyst (LPA) Nicholas Reed conducted an announced Informal Meeting at the Woodland Hills-South Adult and Senior Care Regional Office. RM and LPA met with the former licensee’s husband and sister, Reynaldo Garcia and Marietta Cesicar and disclosed the reason for the visit.
Today’s Informal Meeting was held to discuss future facility operations after the recent death of licensee Yolanda Villanueva. RM Whittaker reviewed the LIC 9117 form which grants emergency approval to operate while new applicants complete their application for a Residential Care Facility for the Elderly.
RM Whittaker inquired about a ‘For Sale’ sign which was observed on the property during a 10/31/25 complaint visit. Reynaldo owns the home and noted there is a waitlist of potential future residents. Therefore, he plans to continue operations with Yolanda’s sister Marietta, who will create an LLC to apply for a new license. The ‘For Sale’ sign will be taken down. Reynaldo also noted Erlina Somera would be the acting administrator. LPA Reed reviewed Erlina’s status on Guardian and the Administrator Certification Bureau’s website and noted that she holds a valid administrator certificate and is currently a licensee at MANOR CARE FACILITY (197603849)
Reynaldo called Erlina at approximately 11:15 a.m. Erlina spoke to the RM over the phone and noted she will be in the Philippines for 3 weeks starting on 11/15/25. Erlina noted Estela will not return from her vacation to serve as a temporary administrator prior to 11/15/25. RM also raised issues noted with day-to-day facility operations noticed during the 10/31/25 complaint visit, especially concerning cooking indoors, accessible gas tanks, expired food, and various clutter throughout the home. RM shared several photos of citable offenses at the facility. RM also requested a list of residents and their representatives from Reynaldo.
