The inspector’s account

Licensing Program Analyst (LPA) Ty Smith and Licensing Program Manager Naira Margaryan conducted unannounced Case Management visit to this facility. The purpose of this visit is to conduct health and safety checks and to verify whether the Administrator and the spouse of the Licensee had contacted all residents’ responsible parties regarding the facility’s closure. Upon arrival, LPM contacted Administrator Estella Agropa by phone and explained the purpose of the visit.

The Administrator confirmed that all families had been informed of the facility closure and stated that she planned to relocate the residents by Sunday, 04/05/26. A few minutes later, Ms. Agropa called LPM again and stated that she was no longer representing the facility and was resigning effective immediately. She also reported that there were three (3) residents currently in care. The daughter of one of the residents was present at the facility, and LPM Margaryan informed her of the facility’s status. She confirmed that she had been notified of the closure and agreed to relocate her mother.

Shortly after the Administrator announced her resignation ,the former Licensee’s spouse and daughter arrived at the facility at 12:25 pm. They informed the licensing staff that all three residents would be relocating to Capps Care Home, LIC# 197610939. They stated that they had already spoken with all responsible parties, and all families agreed to the relocation. According to the Licensee’s spouse, the original plan was to relocate all residents the following day. However, due to the Administrator’s resignation, they decided to proceed with relocation immediately. The Licensee of Capps Care Home arrived during the visit assessed all residents,prepared records,medications for transfer and transferred all residents.

The license has been surrendered, all residents have been moved, and the facility file will be closed as of today: 04/03/26.
