Facility Evaluation Report

Licensing Program Analyst (LPA) Tihesha Smith conducted a case management visit at this facility. LPA Smith was greeted by staff and LPA Smith disclosed the reason for the visit.

The Woodland Hills Regional Office received an SIR reporting that Resident #1 (R1) was a victim of possible sexual abuse by Staff #1 (S1) on 12/09/25. Law enforcement involvement in response to this reported incident was included on the incident report. LPA Smith spoke with the licensee and then R1 from approximately 3:40 to 4:18 pm and obtained documents relevant to the case at approximately 3:55 pm

Due to time constraints a follow-up visit will be conducted at a later date
