Facility Evaluation Report
At around 12:00PM on 1.27.2025 Licensing Program Analyst (LPAs) Leslie Ngo-Castaneda and Nadia Shahbazian conducted an unannounced visit. LPA met with designee, Adeleye Sotimehin, and disclosed the reason for the visit.
During the course of investigating complaint # 31-AS-20250117144918, the staff contacted the administrator Adeleye Sotimehin. LPA explained the allegations and administrator knew why LPA was there for a visit. Although facility staff #6 (S6) and an administrator (S1) observed C1 holding the broom from the backyard sweeping the living room, they did not intervene before C1 attempted to strike Piles and Associates LLC consultant.
Based on Title 22 Regulations, the following deficiencies were cited and recorded on LIC809D.
No immediate health and safety hazard is noted during this visit.
Exit interview was conducted and a copy of report was issued.
