The inspector’s account

Licensing Program Analyst (LPA) Tihesha Smith made an unannounced required annual visit to this f acility at approximately 09:47 am. LPA Smith contacted program manager and checked in with the gate guard. Enroute to the program area, LPA Smith met with Sarah Ramirez (Day Program Manager) and disclosed the reason for the visit. LPA Smith conducted a physical plant tour with Sarah Ramirez and was later joined by Eddie Banuelos. The facility is a day program which operates 9:00 AM -4:00 PM, Monday through Friday. The facility has several client activity rooms on the campus to include a Life Skills room furnished with a bed and other items. There are several shared common patio areas with tables and chairs that are shaded with umbrellas for the clients. The facility does not handle client medications and does not provide meals. The facility does have several vending machines available to clients that include but not limited to refrigerator vending machines, coffee and soda machine. Clients may bring their own meals. There is a café on campus that is not in use located near the patio areas. LPA Smith toured the common areas, classrooms, and observed (4) of the (6) bathrooms. Some of the bathroom faucets are sensor operated. There are first aid kits and fire extinguishers in each activity room including four (4) additional fire extinguishers attached to the outside walls near the patio areas and in café/cooking area. First aid kits were observed to be fully stocked and fire extinguishers observed to be charged. LPA observed clients engaging in and participating in activities within the activity rooms during the visit. Facility grounds were free of hazards. There were no immediate health and safety hazards observed during the day of inspection and no deficiencies cited, however LPA Smith observed a small area of the patio and an activity room cordoned off/or closed. The patio area was cordoned off for general light bulb maintenance, and the activity room was closed due to a flooring tripping hazard identified and a section of tile removed to add a carpeting piece.
