The inspector’s account

At approximately 11:30 a.m. on 03/19/25 Licensing Program Analyst (LPA) Nicholas Reed conducted an unannounced annual visit. LPA met with the administrator and disclosed the reason for the visit.

A file review was conducted prior to today’s visit. The program was last visited on 05/21/2024 for a case management visit. It is a single story campus with fifteen (15) classrooms, four (04) bathrooms, kitchen, café, and outdoor areas. It has an approved fire clearance for ten (10) nonambulatory clients in classrooms.

The campus is gated with security. LPA entered the parking lot through an unlocked gate and met with security at the front. Security provided a name badge and verified LPA’s credentials. LPA toured the facility with administrator Johana Escobar. In the main lobby were postings for personal rights, facility sketch, emergency contacts, COVID precautions, grievance procedure, and the facility license. LPA and administrator toured fifteen (15) classrooms with various purposes such as arts, exercise, cooking, library, and other life skills. Each room had a complete first aid kit and a fully charged fire extinguisher. All fire extinguishers were inspected on 06/10/2024. Two (02) bathrooms were located near classrooms. Two (02) bathrooms were located in the café. Bathrooms contained liquid soap, paper towels, handwashing instruction signs, and grab bars near toilets.

Clients were observed eating lunch at approximately 12:00 p.m. under covered, shaded tables. LPA conducted client interviews at 12:15 p.m. At 1:00 p.m. LPA reviewed the most recent fire safety inspection from 02/22/25 in which all systems passed inspection.

During today's inspection, the facility was in compliance with Title 22 regulations. No immediate health or safety hazards were observed.
