Facility Evaluation Report
On 12/30/22, Licensing Program Analyst (LPA) Melissa Ruiz arrived at the facility to conduct an unannounced annual inspection. Upon arrival, LPA was greeted by the Licensee Saleem Mohammed. Prior to entering, LPA observed covid-19 signage posted outside the front door. LPA was granted entrance, LPA observed hand sanitizer and covid-19 signage posted throughout the facility’s main hallway. Staff were observed to wearing masks. LPA conducted an entrance interview, and the purpose of the visit was explained.
LPA initiated a physical plant tour. Facility is an Adult Residential Facility, vendored by North Los Angeles Regional Center. Facility is licensed for 4 clients. LPA was able to tour the facility and did not observe any immediate health and safety concerns. Sufficient PPE supplies were observed. A fire extinguisher was observed in the kitchen with a date of service of August 31, 2022. Medication is centrally stored and locked in a cabinet. Sufficient food supply was observed. Facility has four (4) bedrooms, two (2) bathrooms. Bedrooms are appropriately furnished and have appropriate lighting. Bathrooms have soap, paper towels and hand washing signs. Extra towels and linens were readily available. There is a covered shaded area in the back yard and there are no bodies of water.
Lastly, during today’s visit, LPA collected additional documents pertaining to Complaint #31-AS-20201112143937.
No deficiencies issued during today’s visit. Report was signed and delivered by Licensee and an exit interview was conducted.
