Facility Evaluation Report

Licensing Program Analyst (LPA) Leslie Ngo-Castaneda conducted a case management visit in conjunction with complaint # 31-AS-20260707153810. LPA did a record review of Client #1 (C1)’s file and an admissions agreement were not available. Per interview with licensee (S1), stated that North Los Angeles County Regional Center (NLACRC) has not given the facility a copy of admission agreement. Although the Regional Center did not provide a copy of the admission agreement or medical assessment, it is the licensee’s responsibility to obtain those per Title 22 Regulations.
