Facility Evaluation Report
On 06/29/26, Licensing Program Analyst (LPA) Gina Saucedo conducted an unannounced annual visit and was greeted by the administrator, Mallory Giacopuzzi. LPA explained the purpose of the visit was to conduct an annual inspection.
The location is a one (1) story building, licensed adult day program. It does have a upstairs for staff use. The Administrator confirmed there are six (6) clients participating in the program.
Upon arrival, LPA observed clients participating in activities with staff supervision.
Kitchen - LPA observed the kitchen contained a refrigerator, sink, and kitchen cabinets. The day progra m have snacks available for the clients. The knives were locked in a cabinet, inaccessible to the clients. Frozen and fresh foods are wrapped and stored appropriately in the refrigerator. There is a fire extinguisher located near the kitchen fully charge and dated May 2026.
There is one (1) bathroom and the water temperature was tested, it was 105.1 degrees F. The bathroom was furnished with paper towels, hand soap, and a trash can. There are chemicals that are locked in an upper cabinet and inaccessible to the clients. There is also incontinence supplies for the clients.
The common area is spacious with proper seating, a table and chairs, a couch, and a television against the wall. There is one (1) main entrance being utilized at the facility. There is a back door that leads to the parking lot area.
