Facility Evaluation Report
Licensing Program Analyst (LPA) Melissa Spaeth conducted an unannounced annual visit and was greeted by the administrator, Mallory Giacopuzzi. LPA explained the purpose of the visit was to conduct an annual inspection. The location is a licensed adult day program to provide services to ambulatory clients. The Administrator confirmed there are seven (7) clients participating in the program.
Upon arrival, LPA observed clients participating in activities with staff supervision. LPA and the Administrator conducted a tour from 10:00 am until 10:20 am. LPA observed the following:
Kitchen - LPA observed the kitchen contained a refrigerator, sink, and kitchen cabinets. The day program have snacks available for the clients. The knives and cleaning supplies were locked in a cabinet. Frozen and fresh foods are wrapped and stored appropriately in the refrigerator. Food storage and preparation areas are clean and inaccessible to pests. The fire extinguisher is located near the kitchen.
Water Temperature - the water temperature was tested at 10:20 am and was 109 degrees F.
Common area - The common area is spacious. LPA observed comfortable chairs, a couch, and tables with various activities available for the clients. There is one main entrance being utilized at the facility.
