Facility Evaluation Report

Licensing Program Analyst (LPA) Alberto Lopez conducted an unannounced visit at the facility for the purpose of conducting CM visit to issue citation for deficiency included in the Eastern Los Angeles Regional Center Annual review. LPA met with H. Lim and discussed the purpose of the visit.

Two issues were addressed during the visit.

1) The air conditioner (AC) was not working. LPA observed the AC working and the home was within the required temperature during this visit. Facility is not required to have AC as long as the temperature does not exceed 85 degrees F

2) Tub appeared to have grime/mold on the caulking around the tub and small spots around the tile.

Deficiency cited on 809D and copy of report, 809D and appeal rights provided.
