Facility Evaluation Report

Licensing Program Analysts (LPAs) Alberto Lopez and Sakinah Madyun made attempted visit on 09/29/2025 and were denied entry.

LPAs arrived at facility unannounced, knocked on door, introduced self and person by the name of "Mila" stated she was instructed to not allow entry to anyone. LPA called and left voice mail for administrator and also sent text message and got no response. "MIla" stated she spoke with Licensee just now and he ask to tell us to make an appointment since facility is closed and has ceased operations.

Licensee denied talking to Mila while LPAs were outside the home. .

Civil penalties issued for repeat violation within one (1) year.

Deficiency noted on 809D and copy of report and appeal rights provided.
