Facility Evaluation Report

Licensing Program Analyst (LPA) made an unannounced visit to conduct annual inspection. LPA met with Licensee Ernie Luna and discussed purpose of the visit. Licensee stated he ceased operations on January 18, 2025. Licensing stated that it was due to HCBS interpretation and excessive documentation by Regional Center that contributed to his decision to cease operations.

LPA took tour of facility and did not observed any clients residing in the home.

Care tool was opened but not used since facility ceased operations.

Licensee handed over the 2 License for the facility to LPA
