Facility Evaluation Report

During the course of a complaint visit LPA measured hot water in 2 restrooms and it measured 125.9 and 129.0 degrees F which is not within range of 105-120 degrees F.

Staff adjusted the water temperature during the visit and it measured 120.2 in one restroom and 129.0 in the second restroom which is still out of range.

Kitchen water temperature was within range.

Also, there is a staff room adjacent to room #4 that is not on facility sketch.
