The purpose of this report is to issue deficiencies that were observed during the 2025 Annual Inspection.
The specific deficiencies to be cited are not located in the Care Tools.
During tour of the facility the following was observed.
In Resident Room 35 sliding door screen has a tear and Room 45 there is a tear in the window screen.
In Room 77 the Resident's mattress had a very large tear in the middle of the mattress.
