Complaint Investigation Report
During interviews with other parties, they stated they do not recall when it happened, there were no witnesses and were unable to provide any additional details. During interviews with staff, all staff stated S1, nor any other staff, has ever physically assaulted C1. All staff were unaware of prior incident.
Based on interviews, there is not enough information to verify the allegation therefore the allegation is UNSUBSTANTIATED at this time.
--- Staff did not report an incident involving resident in a timely manner.
It was alleged that Staff #2 (S2) was immediately informed of the alleged physical abuse incident. To investigate the allegation, on February 26, 2026, LPA interviewed five staff from around 11:30a.m. – 1:00p.m. During interviews with staff, all staff stated they were unaware of the alleged incident and therefore had nothing to report.
Based on interviews, there is not enough information to verify the allegation therefore the allegation is UNSUBSTANTIATED at this time.
--- Resident sustained unexplained injuries due to staff neglect or abuse.
It was alleged that other parties observed multiple bruises on the consumer's body, raising concerns about possible ongoing physical abuse. To investigate the allegation, on February 26, 2026 LPA conducted a physical plant tour at around 10:00a.m., other parties from 11:15a.m. – 11:30a.m. and five staff from around 11:30a.m. – 1:00p.m. During the physical plant tour, LPA did not observe any signs of abuse. All residents were clean and well groomed. During interviews with other parties, they were unable to provide when the bruises were witnessed, who may have done it, any images or other information. During interviews with staff, all staff stated they are unaware of C1 having bruising from any abuse.
Based on observations and interviews, there is not enough information to verify the allegation therefore the allegation is UNSUBSTANTIATED at this time.
No health and safety hazards noted during the visit.
Exit interview conducted and a copy of the report was issued.
