Facility Evaluation Report
On 08/12/25, Licensing Program Analyst (LPA) Regina Cloyd conducted an unannounced Required – 1 Year Inspection to the above-named facility and met with Administrator Lindsey Rae Ackerman and Staff. LPA explained the purpose of the visit and was accompanied by a staff inside and outside the facility during this inspection. This facility is a short-term crisis residential program with an approved fire clearance for six (6) ambulatory adults. The facility has an outstanding balance of Annual Licensing Fees of $681 due on 07/21/2025.
The facility is a two-story house located in a residential street. The home consists of three client bedrooms, three bathrooms, one kitchen, one dining room, one living room, one group room, one therapy office, one nurse office, two balconies, a loft, and a backyard patio area with shaded seating. Outside grounds were toured and no bodies of water were observed. The patio furniture is under a shaded area and accessible to clients. Walkways around the home were clear of hazards. There are no security bars or weapons on the premises.
