Facility Evaluation Report
On 7/3/2024 at around 9:20 AM, Licensing Program Analyst (LPA) Leandro conducted an unannounced Required – 1 Year Inspection to the above-named facility and met with the Executive Director of Operations, Shane Herbert. LPA explained the purpose of the visit and was accompanied by a staff-member inside and outside the facility during this inspection.
This facility is licensed to serve 6 ambulatory adults ages 18 – 59 years.
A total of 5 ambulatory clients are currently residing in this facility.
The facility has an outstanding balance of Annual Licensing Fees of $454 due on 07/21/2024.
The facility is a two-story house located in a residential street. The home consists of 3 client bedrooms, 3 bathrooms, 1 kitchen, 1 dinning room, 1 living room, 1 activity room, 1 office room, 1 nurse office, and a backyard patio area with shaded seating.
