Facility Evaluation Report
At 9:20am, Licensing Program Analyst (LPA) Angela Panushkina arrived at the facility to conduct an unannounced annual inspection. Upon arrival, LPA met with Rebecca Acevedo (S1), who granted access to the facility. Administrator arrived shortly after, and LPA explained the reason for the visit.
At 9:30am LPA conducted a tour of the physical plant and observed the following:
This is a Short-Term Crisis Residential Program, approved for capacity of six (6) Ambulatory clients. LPA was informed that the facility has awake staff at night.
Kitchen: The kitchen area is equipped with a refrigerator, microwave oven and sink. There were adequate supplies of 2-days perishable and 7-days nonperishable food and dining ware to accommodate a maximum capacity of six (6) clients. Sharp knives/objects are kept in the locked cabinet located in a laundry area.
Bedrooms: There are three (3) shared bedrooms designated for clients’ use. All bedrooms are furnished with beds, dressers and required bedding and linen. The bedrooms have sufficient closet space.
Bathrooms: Facility has five (5) bathrooms. Bathrooms have soap and paper towels. Extra towels and linens were readily available. The hot water delivered in the client bathrooms measured at 113.2°F.
Common Areas: The facility maintains a comfortable temperature of 70°F. LPA observed all common areas are furnished with adequate furniture to accommodate a maximum capacity of six (6) clients. There is a functioning telephone on the premises. Dual smoke and carbon monoxide detectors were located throughout the facility, and at 10:00am they were tested and observed to be operational.
