Facility Evaluation Report
At 9:30am, Licensing Program Analyst (LPA) Angela Panushkina arrived at the facility to conduct an unannounced annual inspection. The Administrator arrived shortly after, and LPA explained the reason for the visit.
A tour of the physical plant was initiated at 10:00am and the following was observed:
Facility is licensed for capacity of six (6) Ambulatory clients. LPA was informed that the facility has awake staff at night.
Kitchen: The kitchen area is equipped with a refrigerator, microwave oven and sink. There were adequate supplies of perishable and nonperishable food and dining ware to accommodate a maximum capacity of six (6) clients.
Bedrooms: There are four (4) bedrooms designated for clients’ use, of which two (2) bedrooms (downstairs) are private, and two (2) bedrooms (upstairs) are shared. All bedrooms are furnished with beds, dressers and required bedding and linen. The bedrooms have sufficient closet space.
Bathrooms: Facility has four (4) and half (½) bathrooms. Only two (2) private bedrooms downstairs have a shared bathroom. Client bedrooms upstairs have its own bathrooms. Bathrooms have soap and paper towels. Extra towels and linens were readily available. The hot water delivered in the client bathrooms measured between 105.5 - 107.1°F.
Common Areas: The facility maintains a comfortable temperature of 71°F. LPA observed all common areas
