The inspector’s account

During interviews with staff, all staff stated that they were unaware or did not recall any damage that may have led to flooding a resident’s room #1a. During interviews with residents, Resident #1 (R1) stated there was a leak in one of the resident’s rooms on the back sliding door of the bedroom # 1a. During the visit, LPA did not observed leak in the facility and it was raining on during the visit. All other residents stated they did not know about any flooding in any of the rooms or the facility.

Based on interviews, there is not enough information to verify the allegation. Therefore, the allegation is Unsubstantiated at this time.

Allegation #2: Facility Heating and Air Conditioning system is in disrepair.

It was alleged that the facility is hot at times and cold at times and that the vents are in disrepair. LPA interviewed six (6) out of six (6) staff on who unanimously stated the facility temperature is always comfortable everyday. LPA interviewed four (4) out of the four (4) residents stated the facility is always comfortable. During the visit, when it was raining, LPA observed the thermostat was set to be 69 degrees F and observed the facility vents were open, cleaned, and not clogged. LPA observed that the filter had been replaced. In addition, LPA had administrator turn on the AC and heater to see if it worked. LPA noted that both the AC and heater were operational. During the facility tour at 10:15 am, LPA used an infrared thermometer and observed the facility temperature was 71 degrees F. Based on interviews, there is not enough information to verify the allegation.

Based on records review and observation, there is insufficient evidence to support this allegation. Thus, this allegation is deemed unsubstantiated at this time.

It was alleged the facility is malodorous. Interviews with administrator revealed that they have not smelled any foul odors in the facility. Interviews with six (6) out of six (6) staff revealed have not smelled any odor in the facility. Staff #1 (S1) and staff #6 (S6) revealed facility is cleaned and freshened during and after each shift. LPA observed staff to be cleaning the facility during visit and LPA did not detect any foul odors at time of visit.

The allegation is deemed Unsubstantiated at this time. Exit interview conducted. A copy of this report is given to the administrator.
