Facility Evaluation Report
At 2:30pm, Licensing Program Analyst (LPA) Angela Panushkina arrived at the facility to conduct an unannounced annual inspection. LPA met with the Administrator and explained the reason for the visit.
COMMON AREAS: The facility maintains a comfortable temperature of 73°F. LPA observed all common areas furnished with adequate furniture to accommodate a maximum capacity of six (6) clients.
KITCHEN: The facility has a kitchen area that is equipped with a refrigerator, microwave oven and sink. LPA observed there to be sufficient stock of one-week perishable foods and two-day non-perishable foods. Frozen foods are properly wrapped and stored appropriately. Food storage and preparation areas are clean and inaccessible to pests. The fire extinguishers are located in the kitchen, group room and in a hallway and were last serviced on 02/03/2026.
LAUNDRY ROOM: The laundry room is located by the kitchen. LPA observed the washer/dryer appeared to be in good condition. Laundry supplies are kept inaccessible when not in use with supervision.
THERAPY/TREATMENT AREAS : Group Therapy is conducted in the common areas.
BEDROOMS: The facility has six (6) private bedrooms designated for clients use. All bedrooms are furnished with beds, dressers and required bedding and linen. The bedrooms have sufficient closet space. Facility has awake staff at night.
BATHROOMS: The facility has six (6) bathrooms and LPA observed all bathrooms have non-skid mat, soap and paper towels. Extra towels and linens were readily available. The hot water delivered in the client bathroom measured at 117.3°F.
