Regarding the Allegation : Staff do not assist clients with toileting needs.

This complaint alleged that the facility staff did not help C1 go to the bathroom. LPA Calderon noted staff giving morning medications to clients. LPA Calderon did not witness any negative interactions between staff and clients. Records review indicate the following: Physician Report and Needs and Service Plan indicate that C1 does not need any help going to the bathroom. Interviews indicate the following: 4 out of 4 staff deny the allegation. C1 indicates that staff do not have enough pull ups for sleeping at night. C1 indicates that C1 does not need any help going to the bathroom. 6 out of 7 clients deny the allegation.

Based on interviews and supporting documentation, the preponderance of evidence standard has NOT been met therefore, the allegation of “staff do not assist clients with toileting needs” is found to be UNSUBSTANTIATED.

Regarding the Allegation : Staff allow clients to be left in soiled clothing for extended periods. This complaint alleged that the facility staff left C1 in dirty clothes. LPA Calderon toured the facility with S1 and did not notice any negative interaction between staff and clients. LPA Calderon viewed the commercial washing machines being used by staff. LPA Calderon witnessed clients picking up washed and folded clothes. 4 out of 4 staff deny the allegation. C1 indicates that staff do wash C1 clothes, but unknown invisible people steal C1 clothes and leave dirty clothes for C1 to find. 6 out of 7 clients deny the allegation.

Based on interviews and supporting documentation, the preponderance of evidence standard has NOT been met therefore, the allegation of “staff allow client to be left in soiled clothing for extended period of time” is found to be UNSUBSTANTIATED.
