Facility Evaluation Report

On April 30, 2025, Licensing Program Analyst (LPA) Bernadette Allen conducted an unannounced visit to assess the health and safety of the residents in care. Upon arrival, LPA met with Administrator Mary Montiano and explained the purpose of the visit.

At 9:15 AM., LPA requested and received files for both staff members and residents. At 11:45 AM. a comprehensive tour of the facility was conducted, during which LPA observed ten (10) bedrooms, all equipped with the required furniture in good condition, and three (3) bathrooms that were clean and fully operational. LPA also toured the kitchen and food storage areas, observing a (5) five day supply of perishable food items and a (7) seven day supply of non-perishable items.

During the inspection, LPA Allen obtained the necessary documents and did not identify any health or safety concerns. No deficiencies were cited during this visit.

An exit interview was conducted where this report was discussed and provided to Mary Montiano- Administrator at the conclusion of the visit.
