Facility Evaluation Report
On 3/08/2024 at around 11:40 AM, Licensing Program Analyst (LPA) Socorro Leandro conducted an unannounced Required – 1 Year Inspection to the above-named facility and met with the Administrator Mary Jane Montiano. LPA explained the purpose of the visit and was accompanied by the Administrator inside and outside the facility during this inspection.
This facility is licensed to serve 68 adults ages 18 – 59 years, of which 50 may be non-ambulatory clients. This facility is approved for 5 hospice clients.
A total of 54 clients are currently residing in this facility.
The facility is composed of 2 buildings located in a residential street.
The Main Building is composed of 22 client bedrooms, 1 laundry room, 1 dining room, 4 large bathrooms, 1 industrial kitchen, several toilet rooms, main office, lobby, storage rooms, several office spaces, and 1 activity room.
The Annex Building is composed of 15 client bedrooms, 3 bathrooms, office spaces, and an employee lounge.
Outside grounds consist of 2 parking lots and patio areas with shaded seating.
