Facility Evaluation Report
On 03/17/26, Licensing Program Analyst (LPA) Elvira Gonzalez conducted an unannounced annual required visit using the CARE Inspection Tool. LPA met with staff, Angelina Montes, and the purpose of the visit was explained. The facility is licensed to serve for four (4) developmentally disabled adults ages 18-59, and is approved for four (4) ambulatory of which two (2) may be non-ambulatory. There are currently four (4) clients in the facility.
The facility is a single story home located in a residential neighborhood. The home consists of three (3) bedrooms, two (2) bathrooms, living room, office area, kitchen, dining room, a detached garage that is used for storage, houses a washer and dryer and a deep freezer, and outside shaded seating available in the back yard.
LPA Gonzalez toured the inside and outside of the facility. All client rooms were checked. Mattresses and box springs were in good condition, adequate lighting was observed, plenty of dresser and closet space was observed. Walls and floors were clean and in good repair. Bed linens, comforters and bath towels were adequately stocked at the time of visit.
Bathrooms were checked and found to be within Title 22 regulation. Toilets and water faucets worked properly. Shower was free of mold/mildew, there was adequate lighting, and sufficient toiletries accessible to clients. The water temperature properly measured between 105-120 F.
