The inspector’s account

On 04/23/26, Licensing Program Analyst (LPA) Sparkle Day conducted an unannounced annual required visit using the CARE Inspection Tool. LPA met with the Assistant Administrator, Itzia Maciel and the purpose of today’s visit was explained. The facility is licensed to operate for 106 Adults ages 18-59 years of age of which 106 maybe non ambulatory. The facility has an approved Hospice Waiver for (25) clients. Currently, the home has (104) clients.

The facility is a one-story building that consists of 53 client bedrooms, 4 shower rooms, 1 industrial kitchen, several toilet rooms, 1 dining room, 2 court yards, 1 TV room, several office spaces, 1 parking lot, 1 laundry room, 1 activity room, 1 break room, and an outside seating areas.

LPA conducted a records review of (6) client records, (6) staff records, (6) clients Personal & Incidental Records and reviewed the facility disaster plan. All client & Staff records were complete. The facility disaster plan was current and in compliance with Title 22 at the time of visit. LPA reviewed (6) Client Medication Administration Records and did not observe any discrepancies at the time of visit. Last Fire Inspection LAFD 3/12/26.

At 10:00am LPA and Asst Administrator toured the inside and outside of the facility. Seven (7) client rooms were checked. Mattresses and box springs were in good condition, adequate lighting was observed, plenty of dresser and closet space was observed. Walls and floors were clean and in good repair. Bed linens, comforters and bath towels were adequately stocked at the time of visit. Bathrooms were found to be within Title 22 regulation. Toilets and water faucets worked properly.
