On 05/24/23, Licensing Program Analyst, (LPA) Mario Leon conducted a Case Management visit after observing rodent droppings in two (2) locations in the facility kitchen. One (1) pile of droppings were observed in the Southwest corner and the other was near the center, below the kitchen counter, of the Southern wall.
LPA informed Administrator (S1), and S2, of the findings and explained the purpose of this case management visit.
