The inspector’s account

On 05/09/2025 around 2PM, in Facility Highland Manor, Licensing Program Analyst (LPA) Jose Anguiano conducted a case management visit and an interview with (Administrator) Aaron Khodorkovsky. (LPA) Anguiano documented deficiencies related to an Unusual Incident report taken by taken by the department, LPA explained the purpose of the visit to the Administrator. Interviews conducted and records reviewed revealed that the Licensee did not prevent facility to be free from corporal or unusual punishment.

Deficiencies were observed via Staff (S1) pushing Client (C1) on May 8th, 2025. LPA Anguiano explained the purpose of the visit to administrator; interviews were conducted and records reviewed. Observation in accordance with the California Code of Regulations, Title 22. A copy of appeal rights and this report was left with the Administrator Assistant Itzia Maciel.

An exit interview was conducted a copy of the report and appeals were provided.
